What is Perinatal & Postpartum Therapy?
Perinatal therapy is specialized mental health support for individuals and couples during fertility, pregnancy, postpartum, and early parenting. We help you work through mood changes, relationship challenges, identity shifts, and emotional overwhelm, so you don’t have to carry it all by yourself.
This includes what’s often called matrescence, the psychological and identity shift that comes with becoming a mother. It’s a normal developmental passage, not a diagnosis, but it can still bring real grief for who you were before, ambivalence about your new role, and a changed sense of who you are. Our therapists help you hold both the love and the loss that can come with this transition.
- Anxiety or depression during pregnancy
- Postpartum anxiety
- Postpartum depression
- Persistent guilt, self-doubt, or perfectionism
- Trouble bonding with your baby
- Communication breakdowns with your partner
- Emotional grief related to fertility or loss
- Feeling overstimulated or reactive to your child’s emotions
Only 10-15% of those needing perinatal therapy receive it resulting in longer wait times. And if you’re a women with ADHD we understand how this can lead to a particular kind of pressure or overwhelm as a mom. We offer Perinatal Mental Health Counselling and Postpartum Therapy in-person at our Toronto clinics or online across Ontario both weeknights and weekends, with no wait list.

Approaches We Use in Perinatal & Postpartum Therapy
We use evidence-based therapies tailored to your unique experiences, emotions, and goals as a parent. These approaches support healing, connection, and growth throughout every stage of the perinatal and postpartum journey:
I-I Perinatal and Postpartum Therapy
This integrative approach blends evidence-based techniques with a focus on the unique emotional, relational, and identity shifts that come with parenthood. It supports the full journey, from trying to conceive to parenting, while honouring your lived experience.
Dialectical Behaviour Therapy (DBT)
DBT helps you manage intense emotions, improve distress tolerance, and reduce reactivity, especially helpful for postpartum mood swings and overwhelm. It teaches practical tools to regulate emotions and respond more calmly to parenting stress.
Acceptance and Commitment Therapy (ACT)
ACT helps you accept difficult thoughts and feelings without judgement, while staying committed to your values as a parent. It’s especially useful for working with self-doubt, perfectionism, and identity changes in early parenthood.
Emotion Focused Therapy (EFT)
EFT is often used in couples therapy to deepen emotional connection and improve communication. In the perinatal period, it helps partners navigate the emotional impact of fertility struggles, pregnancy, and parenting together.
Internal Family Systems Therapy (IFS)
IFS helps you explore and heal the inner parts of you that may be critical, anxious, or shut down, particularly those activated by new roles and responsibilities in parenthood. It supports self-compassion and emotional integration.
Cognitive Behavioural Therapy (CBT)
CBT addresses unhelpful thinking patterns like catastrophizing, guilt, or self-criticism that can arise during fertility struggles or postpartum. It helps you reframe thoughts and develop more balanced, supportive beliefs.
Psychodynamic & Relational Therapy
This approach explores how past experiences and relationships shape how you view yourself and your role as a parent. It supports deep emotional insight and healing, especially if parenting brings up unresolved wounds.
Mindfulness & Attachment-Based Therapy
Mindfulness builds present-moment awareness and self-regulation, while attachment-based therapy helps you understand and respond to your child’s emotional needs. Together, they support bonding and reduce reactive parenting.
Polyvagal Theory
Polyvagal-informed therapy works with your nervous system to help you feel safe, connected, and regulated. It’s especially helpful for calming postpartum anxiety, trauma responses, and feeling “on edge” as a new parent.

Questions About Perinatal & Postpartum Therapy
How do I know if I have postpartum depression or if I’m just exhausted from having a baby?
Many new parents experience fatigue and emotional ups and downs. Postpartum depression often involves persistent sadness, anxiety, irritability, or feeling disconnected from the baby or yourself. If these feelings continue for several weeks or feel overwhelming, speaking with a therapist can help.
How will perinatal or postpartum therapy help me?
Therapy can support you through the many emotional stages of planning for and building a family. Whether you’re trying to conceive, navigating pregnancy, or adjusting after birth, support is tailored to your unique needs. At Horizon Therapy in Toronto, we draw from effective approaches including Emotion-Focused Therapy, Dialectical Behaviour Therapy (DBT), Acceptance and Commitment Therapy (ACT), Cognitive Behavioural Therapy (CBT), Polyvagal Theory, and Mindfulness. You’ll have space to explore your concerns, address anxiety or depression, build confidence in your abilities, and learn new self-care tools and attachment-based parenting strategies.
What is matrescence?
Matrescence is the term for the psychological and identity transformation that happens when someone becomes a mother. First coined in the 1970s and gaining renewed attention today, it describes a normal developmental passage rather than a mood disorder, though it can still involve real grief for your former life, ambivalence about your new role, and a changed sense of who you are. Matrescence is different from postpartum depression or anxiety, since it isn’t about a clinical symptom to treat, but about supporting the identity work of becoming a parent. Many people experience both at once, and therapy can help with either or both.
Why does motherhood feel lonely even when I’m never alone?
Many new parents feel isolated even when surrounded by people. Motherhood can change your daily routine, priorities, and social connections, leaving less time for adult conversation, support, or activities that bring joy. Feeling lonely is a normal experience and doesn’t mean something is “wrong” with you. Therapy can help you process these feelings, find connection, and build supportive networks that validate your experiences as a parent.
Why do I feel anxious or constantly worried after having a baby?
The different stages of family-building (pregnancy, postpartum, and beyond) can bring emotional highs and lows. Postpartum anxiety is very common and can include racing thoughts, constant worry about the baby’s safety, or difficulty relaxing. Hormonal changes, sleep deprivation, and the stress of adjusting to parenthood can all contribute.
If feelings of sadness, irritability, anxiety, or disconnection persist for more than a couple of weeks or begin to affect your daily life, it may be a sign of postpartum depression or postpartum anxiety. Reaching out for support is a strong and healing first step.
Why don’t I feel the instant bond with my baby that everyone talks about?
Bonding can take time. Many parents feel pressure to experience immediate connection, but emotional attachment often develops gradually as parents and babies learn about each other.
Can I still bond with my baby if I have postpartum depression or anxiety?
Yes! Bonding doesn’t require perfection. It’s about showing up for your baby, even when you’re not feeling your best. If you’re struggling with postpartum anxiety or postpartum depression, therapy can help you find your way back to connection. With support and consistency, your confidence and your bond can grow stronger over time.
Why do I feel angry or irritable after having a baby?
Postpartum anger and irritability are common and often linked to hormonal changes, sleep deprivation, stress, and the huge adjustment of caring for a new baby. Feeling frustrated doesn’t mean you don’t care about your child, it’s a natural response to exhaustion and overwhelm. A therapist can help you identify triggers, practice self-care, and develop strategies to regulate emotions and communicate more effectively with your support system.
Is it normal to regret becoming a parent sometimes?
Yes. Many parents experience occasional regret or doubt, especially in the early months when sleep deprivation, hormonal changes, and the demands of caring for a newborn are overwhelming. These feelings do not mean you are a bad parent or that you don’t love your child. Therapy can provide a safe space to explore these emotions, reduce guilt, and develop coping strategies so you can navigate parenthood with more confidence and support.
How long will it take to feel better?
There’s no one-size-fits-all answer, because each person’s healing process is different. That said, many clients begin to feel lighter, more self-aware, and better able to cope within the first few months of regular therapy. Whether you’re dealing with infertility stress, new parenthood, or relationship challenges, therapy offers a space to feel seen and supported. “Everyone wants to hold the baby, but who holds the mother? We do, here at Horizon Therapy.”
Can therapy help me with infertility?
Yes. Infertility counselling can be a crucial support during what is often a stressful and emotionally exhausting journey. Our Toronto perinatal and postpartum therapists walk alongside you as you navigate fertility cycles, medical appointments, uncertainty, tough decisions, and the pressure of day-to-day life. You don’t have to carry this alone; we’re here to help you feel more grounded and emotionally supported.
